Patient Intake Paperwork

In an effort to provide you the most efficient care, while our Nurse Practitioners are awaiting credentialing by your insurance provider, you may wish to consult with them earlier to determine if medication management is appropriate and begin treatment. There is currently NO WAIT for cash pay clients. Typically, an initial evaluation with a Psychiatric Nurse Practitioner is an in-depth, at least one hour in length appointment. Follow-up appointments are generally scheduled for one month later, once the medication has reached its full therapeutic value to determine if increase, adjustment, different or additional medications are needed.
